Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while using improperly maintained mobile ladder stands.   a) Shop - on or about February 11, 2015, a nine riser mobile ladder stand being used by production employees was not properly fitted with the rubber safety stops required to prevent movement when the ladder was in use.  ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement method to correct this hazard is: Comply with the manufacturer's requirements and ANSI A14.7-1991 for Mobile Ladder Stands and discontinue use until repairs are completed. Units that are damaged or weakened from any cause shall not be used until repairs are completed.  Units that are damaged subsequent to their receipt, and/or are worn beyond repair shall be destroyed.  Never climb a damaged ladder stand or ladder.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a) Shop - on or about February 11, 2015, employees working from an elevated working surface, being used to store supplies approximately nine feet, six inches above a lower level, were not guarded with standard railings or any other means of fall protection.
