29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: CNC Milling Area: On or before February 26, 2015, and at times prior thereto, employees were exposed to a caught-in hazard from moving equipment while entering inside a Yama Seiki CNC machine (Model: BM 1600 Serial: 13024) to change rotating tables and tighten bolts without isolating (Lock out Tag Out) the disconnect switch to prevent unexpected startup of the machine.

(b) CNC Milling areas: YCM CNC (Model: MV 76A Serial: 273), On or about 26 February, 2015, and at times prior thereto, employees were exposed to a struck-by hazard while using a rotating edge finding tool and a cutting tip to mill metal parts with the door open (guard bypassed) on CNC machines that had the door safety interlock switch bypassed allowing the door to be opened during operation.
