29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  a/ Tory Burch/Monroe Twp. NJ Site: Warehouse. The temporary agency employer did not develop, document and certify a hazard assessment to identify potential hazards temporary employees would be exposed to. Temporary employees were hired to work at Tory Burch LLC where they were exposed to hazards associated with (including but not limited to) handling packages/boxes, operations involving powered industrial trucks, baler and conveyor equipment.  Observed on/or about 2/25/2015.   The Infinity Recruiting Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard  29 CFR 1910.132(d)(2), which was contained in OSHA inspection number 995869, citation number 1, item number 2 and was affirmed as a final order on January 15, 2015, with respect to a workplace located at 601 Lehigh Ave., Union, NJ 07083.
