Employees were performing a roofing job located at 1400 Boswell in Topeka, Kansas, the ground man was exposed to head injury from falling objects from the roof.

Employees performing a residential roofing job at 1400 Boswell in Topeka, Kansas were exposed to fall hazards of 14 feet at the lowest eave to more than 22 feet. The employees were working on the 8 x12 pitch roof with no fall protection in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Employees were performing a residential roofing job located at 1400 Boswell in Topeka, Kansas; employees using the portable extension ladder to access the roof were exposed to fall hazards of approximately 14 feet from the roof to the surface below. a) Extension ladder on the east porch of the structure. The side rails extended approximately 1 foot 6 inches above the landing surface of the roof. b) Extension ladder on the north-east corner of the structure. The side rails extended approximately 2 foot 6 inches above the landing surface of the roof.
