29 CFR 1910.106(e)(6)(i):  Adequate precautions were not taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.    On or about 02/25/2015, at the above addressed location, employees were exposed to burn injuries from gasoline and oil storage tanks which were not protected from sources of ignition.

Spills shall be cleaned up promptly.  On or about 02/25/2015, at the above addressed location, employees were exposed to a fire hazard from leaking gasoline and diesel fuel tanks which created puddles of fuel in the pit under the tanks.

On or about 02/25/2015, at the above addressed location, employees were exposed to electrocution hazards from an electrical wires inside a conduit that had separated adjacent to a gasoline storage tank.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:     a) On or about February 25, 2015, at above addressed job site, the employer did not secure propane gas cylinders to prevent them from being knocked over.     b) On or about February 25, 2015, at above addressed job site, the employer's propane gas tanks were observed without caps.     H&R Paving, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.101(b) which was contained in OSHA inspection number 801742, citation number 01, item number 01 and was affirmed as a final order on 07/12/2013, with respect to a workplace located at 1955 NW 110 Avenue, Miami, Florida 33172.
