29 CFR 1910.26(c)(3)(iii): Ladders were used on surfaces which did not provide a secure footing: 1050 Westminster Street, Fitchburg, MA: On or about February 25, 2015: employees were observed using a portable ladder to access a roof which was set on top of a snow pile which did not provide a secure footing exposing employee to falls from height.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: 1050 Westminster Street, Fitchburg, MA: On or about February 25, 2015: employees were observed shoveling snow from a roof over 14 feet above the next lower level and PPE was not used exposing employees to falls. United Industrial Roofing, INC. was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(10), which was contained in OSHA inspection number 910723 , citation number 1, item number 1 and was affirmed as a final order on August 19, 2013, with respect to a workplace located at 2101 Commerce Drive Bridgeport, CT 06606.
