29 CFR 1903.19(c)(1): Within ten calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section:  a.  The employer has failed to submit abatement certification within ten days of the abatement date, May 2, 2015, for a violation of 29 CFR 1904.29(b)(1) issued as citation 2, item 1, and described as follows:      For the calendar year 2015 OSHA 300 log, the employer had not included all the information in column F as required for the following entries:    1. Case no. B did not describe the injury that the employee received, did not plainly describe which leg suffered the injury, and the injury was not classified and no days away from work or job transfer/restriction or injury type were not filled out.     2. Case no. C did not describe the injury that the employee received, did not plainly describe which leg suffered the injury, and the injury was not classified and no days away from work or job transfer/restriction or injury type were not filled out.

29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a. On or before January 4, 2015, the highest ranking official at the company did not certify the OSHA form 300A for the calendar year 2014.    Universal Fabrications and Construction Services, dba UNIFACS was previously cited for a violation of this occupational safety and health standard, 29 CFR 1904.32(b)(3), contained in OSHA inspection number 578359, citation number 2, item number 1, and was affirmed as final order on May 7, 2014, with respect to a workplace located at 1241 McKinley Avenue, Columbus, Ohio.

A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete:  a. On or before January 4, 2015, the highest ranking official at the company did not certify the OSHA form 300A for the calendar year 2014. You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.
