29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   The employer does not guard pulleys which are 7 feet or less from the floor or working platform. This violation was observed on or about February 26, 2015, on the air compressor where employees are exposed to a caught-in hazard when walking adjacent to operating the compressor without having a guard on the pulleys.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all pulleys 7 feet or less from the ground are guarded.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:   The employer does not guard belts which are 7 feet or less from the floor or working platform. This violation was observed on or about February 26, 2015, on the air compressor where employees are exposed to a caught-in hazard when walking adjacent to operating the compressor without having a guard on the belt.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all belts 7 feet or less from the ground are guarded.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   The employer does not effectively close unused openings in breaker cabinets where there are unused openings. This violation was observed on or about February 26, 2015, in the right electrical panel where employees are exposed to an electrical hazard when accessing the breaker panel without blanks being installed where there are unused openings.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all unused openings are effectively closed.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer does not protect the operator and other employees in the machine area from hazards such as those created by rotating parts and flying chips. This violation was observed on or about February 26, 2015, in the production shop where employees were exposed to caught-by and struck-by hazards when operating the Big Joe lathe without the chuck of the lathe being guarded to protect the operator from rotating parts and flying chips.    Deco Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 914124, citation number 1 item number 2 and was affirmed as a final order on August 9, 2013, with a final abatement date of August 1, 2013, with respect to a workplace located at 1519 Hill Rd., Houston, TX  77039.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that operators and other employees in the machine area are protected from hazards such as those created by rotating parts and flying chips.
