29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about February 26, 2015, at the above addressed jobsite, an employee was exposed to a 7-foot fall while working on top of the edge of the truck wagon.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  On or about February 26, 2015, at the above addressed jobsite, an employee was exposed to a fall hazard while working from a ladder that had missing anti-slip feet, broken side rails, and had not been inspected by the employer.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about February 26, 2015, at the above addressed jobsite, and employee was working from a ladder which had broken/missing  slip resistant feet and side rails.
