29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) On or about February 11, 2015, while clearing a jam on the Trumpf 5000 punch press, located in Fabrication Shop One at 873 Georgetown Rd., Salem, Ohio 44460, the employee did not lock or tag out the press or utilize the service key before entering the area of the equipment to clear the jam.    b) On or about February 26, 2015, while setting up the Trumpf 5000 punch press, located in Fabrication Shop One at 873 Georgetown Rd., Salem, Ohio 44460, the employee did not lock or tag out the press or utilize the service key before entering the area of the equipment to set the dies in place.    c)  On or about February 26, 2015, while setting up the 400 ton Accupress punch press, located in Fabrication Shop One at 873 Georgetown Rd., Salem, Ohio 44460, the employee did not lock or tag out the press or utilize a die block before changing the dies.
