29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw. (a) Trailer - On or about February 24, 2015 an employee was exposed to amputation hazard when using two Ryobi CSB134L Circular Saws without the guards in place. One saw guard was blocked by a small section of wood. The other saw guard was tied back with string. Employees using saws were exposed to contacting rotating saw blades because lower saw guards were blocked.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) Building C - On or about February 24, 2015 an employee was exposed to a fall hazard while working in a JLG-600 Series Telescopic Boom Lift (serial no. 481-60-7288). Employee was elevated without wearing body belt and lanyard.
