A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times: a. On or about February 23, 2015, in DZ Lane 05 of the warehouse, an employee who was operating a Crown RC 3000 Series stand-up electric forklift (S/N: 1A293030) did not keep the truck under control at all times when the employee hit the protective barrier around a structural column which caused injuries resulting in hospitalization.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a. At the workplace, the employer did not ensure that the refresher training and evaluation were certified for an employee who had been observed operating a powered industrial truck in an unsafe manner, in that the pre-operation checklist was not completed, on or about February 24, 2015. c. At the workplace, the employer did not ensure that the training certifications included the identity of the person performing the training. d. At the workplace, the employer did not ensure that any evaluations of operators on the powered industrial trucks were certified.
