OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. (a) Service King Parking Lot - On or about March 23, 2015 an employee was exposed to a fall hazard while using a fiberglass step ladder. This employee was standing on the top cap of the step ladder. This ladder was placed on top of a van 12.2 feet above concrete. (b) Service King Parking Lot - On or about March 23, 2015 an employee was exposed to a fall hazard while using a fiberglass step ladder that was not placed with a secure footing on a firm, level supporting surface. This ladder was placed on top of a van 12.2 feet above concrete.
