29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a). Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employee had climbed out on to the top of the parapet wall and was not protected from falling by the use of any means of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a). Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employee exposed to fall hazards had not been trained to recognize and procedures needed to minimize fall hazards.

29 CFR 1926.1053(b)(7): Ladder that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:   (a). Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the roof structure was placed on ice and was not secured at the top or bottom of the ladder to prevent slips.
