29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: (a) Building E - On or about February 24, 2015 employees were exposed to a fall hazard of 12 feet 6.5 inches to ground. The scaffold was not fully decked or planked. Scaffold had a 24 inch gap in the scaffold platform.
