29 CFR 1910.119(d)(3)(ii) The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a)Mahantango Site, main ammonia engine room, the door leading from the engine room interior to the operator control room was not self closing and the employer did not document that the equipment complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. b)Mahantango Site, main ammonia engine room, a household window air conditioner was mounted in the wall between the engine room and the operator control room and directed engine room air directly into the control room and the employer did not document that the equipment complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. c) Mahantango Site, O Cellar, two tube and shell heat exchangers were located in an occupied process and production area and an ammonia detection system was not installed to provide employees with an early warning in the event of a leak, on or about June 18, 2015. 29 CFR 1910.119(j)(4)(ii) Inspection and testing procedures shall follow recognized and generally accepted good engineering practices: a)Mahantango Site, O Cellar, the ammonia to glycol tube and shell heat exchanger had distorted/broken outer aluminum jacket, disturbed insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. b)Mahantango Site, ammonia / carbon dioxide engine room, piping and appurtenances to the ammonia to glycol heat exchanger had missing insulation, visible corrosion, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. c)Mahantango Site, main ammonia engine room, compressor #5 ammonia had an inlet elbow assembly which had no insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. d)Mahantango Site, main ammonia engine room, piping and tee as part of the pump receiver vessel installation which was located adjacent to compressor #6 had missing insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. 29 CFR 1910.119(l)(1): The employer did not establish written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: a)Mill Creek Site, an electrical generator set, powered by a methane fueled internal combustion engine, was installed near the anhydrous ammonia engine room, and the changes to the process and equipment, were not addressed, on or about, February 27, 2015. b)Mahantango Site, O Cellar, piping and appurtenances to a tube and shell heat exchanger were changed from a motorized and remotely controlled valve to a manual valve and the changes to the process and equipment were not addressed, on or about, June 18, 2015. c)Mahantango Site, main ammonia engine room, a new compressor was added to the system and the changes to the process and equipment were not addressed, on or about, June 18, 2015. d)Mahantango Site, a new condenser was added to the system and the changes to the process and equipment were not addressed, on or about, June 18, 2015.
