29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   The employer does not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure before employee performed any servicing or maintenance on a machine or equipment where the energizing, startup, or release of stored energy could occur and cause injury. This violation occurred on or about February 25, 2015, and times prior thereto, where employees in the shop area were exposed to struck-by and electrical hazards when performing service and maintenance on industrial machinery without an established program consisting of energy control procedures, employee training and periodic inspections.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  a program is established consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   The employer does not adjust work rests on grinding machinery to one eight inch maximum opening. This violation was observed on or about February 25, 2015, near the west overhead door where the employees were exposed to a struck-by hazard when operating a Dayton 8 inch bench grinder without work rest(s) adjusted to a maximum of one eight inch.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rest(s) on grinding machinery are adjusted closely to the wheel with a maximum opening of one eight inch.

This violation was observed on or about February 25, 2015, near the west overhead door where the employees were exposed to a struck-by hazard when operating a Dayton 8 inch bench grinder without tongues guards being adjusted to a maximum of one fourth inch.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rest(s) on grinding machinery are adjusted closely to the wheel with a maximum opening of one eight inch.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  The employer does not ensure that unused openings in electrical boxes are effectively closed. This violation was observed on or about February 25, 2015, in the NE corner of the shop where employees were exposed to an electrical hazard when accessing an electrical panel without unused openings being effectively closed.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that unused openings in boxes, cabinets, or fittings are effectively closed.
