29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a.) National Machinery, LLC located at 161 Greenfield St. Tiffin, OH 44883; On or about February 26, 2015, the employer did not assure employees were protected while changing tools on the Kolb 2 CNC machine. On November 7, 2014, an employee was able to enter the gated area of the tool changer while in operation and received a crushed middle and ring finger between two tools.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) National Machinery, LLC located at 161 Greenfield St. Tiffin, OH 44883; On February 21, 2015, the employer did not assure employees were protected while using the Cincinnati Shear in the Fabrication Department. An employee was required to use the shear to trim down metal sheets and received a partial amputation of the right middle finger from the first stomper after reaching under the guard.
