29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) In the workshop, on or about February 26, 2015, and at times prior to, the end of an electrical cord to run the hinge boring machine was detached from the plug, allowing strain to be placed directly on the energized conductors, exposing employees to electrical shock hazards.  Employees use the hinge boring machine to build cabinets.
