29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about February 26, 2015, at the Holy Family Apartments renovation site, the employer did not ensure that flexible cords (extension cords) used to power the compressor and staple gun were fit for use in wet conditions. The extension cord coming from the temporary power panel (orange cord) had four (4) severe abrasions which exposed the inner wires and their insulation to damage and employees to contact with the conductors.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap: On or about February 26, 2015, at the Holy Family Apartments renovation site, the employer failed to ensure that the power cord of the air compressor was maintained in a continuous length. The cord was spliced into an extension cord such that the conductors were not properly insulated and the outer sheath was missing at and around the splice.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Instance 1: On or about February 26, 2015, at the Holy Family Apartments renovation site, the employer did not ensure that the strain relief was maintained on the cord and plug of the extension cord coming from the temporary power panel such that the outer insulation was pulled away at both ends of the cord. Instance 2: On or about February 26, 2015, at the Holy Family Apartments renovation site, the employer did not ensure that the strain relief was maintained on the cord and plug of the extension cord plugged into the cord coming from the temporary power panel such that the outer insulation was pulled away at one end of the cord.
