29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Grand Ronde Apartment Complex- (a) The power cord of DeWalt right angle drill was missing the ground prong and had not been inspected prior to use. (b) The power cord of DeWalt, DMW120, band saw was missing the ground prong and had not been inspected prior to use. (c) The 40 ft. yellow extension cord was missing the ground prong and had not been inspected prior to use.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. (a) The yellow 40 ft. extension cord was being used with no ground, exposing employees to electrical shocks. (b) The DeWalt right angle drill was being used with no ground, exposing the employee to electrical shocks. (c) The portable DeWalt, DMW120, band saw had no ground, exposing the employee to shocks.
