29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  a) 5R Processors, Ltd., Lithonia, GA - Employees worked in the distribution warehouse with excessive high boxes and equipment storage. In the back of the warehouse (Scrap Area), the exit door was padlocked, blocked with boxes and equipment, and the exit sign was not illuminated, on or about 01/28/15.  a) 5R Processors, Ltd., Lithonia, GA - Employees worked in the distribution warehouse with excessive high boxes and equipment storage. In the Retail Room, the exit door was padlocked, and the exit sign was not illuminated, on or about 01/28/15.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  5R Processors, Ltd., Lithonia, GA - An employees was assigned the operation of the Yale propane powered sit down forklift in the distribution warehouse. The employee did not receive formal instruction or practical training, on or about 01/28/15.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:  5R Processors, Ltd., Lithonia, GA - Forklift operators were assigned the operation of the sit down forklift in the distribution warehouse. The horn of the Yale propane powered sit down forklift was not working, on or about 01/28/15.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Under section 17(c) of the Occupational Safety and Health Act of 1970 and 29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) 5R Processors, Ltd., Lithonia, GA - Back exit door - Scrap Area - Employees were exposed to an entrapment hazard.   The emergency exit door was obstructed with excessive boxes along with large and heavy storage materials, making it difficult for employees to escape in the event of an emergency, on or about 01/28/2015.   The 5R Processors, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1006202, citation number 1, item number 1a and was affirmed as a final order on 01/02/15, with respect to a workplace located at 2445 Stone Mountain-Lithonia Road, Lithonia, GA 30058.

Under section 17(c) of the Occupational Safety and Health Act of 1970 29 CFR 1910.305(d): Panelboards that were mounted in cabinets, cutout boxes, or enclosures approved for the purpose were not of the dead-front type:  a) 5R Processors, Ltd., Lithonia, GA - Technician Area - Employees were exposed to an electrical shock hazard. The wall mounted panel board/circuit breaker was not equipped with a dead front and the front access panel door was removed from the electrical equipment, exposing employees to live parts when accessing and/or walking, and working next to the unguarded electrical circuit breaker, on or about 01/28/15.      The 5R Processors, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(d), which was contained in OSHA inspection number 1006202, citation number 1, item number 3e and was affirmed as a final order on 01/02/15, with respect to a workplace located at 2445 Stone Mountain-Lithonia Road, Lithonia, GA 30058.
