Throughout the Workplace - On or about and times prior to February 25, 2015, a Hyster lift truck (model:80) was operated with a non-functional horn and reverse alarm, was missing the locking mechanism used to secure the propane cylinder inside the rack, and a damaged left fork, thus exposing employees to struck-by hazards.    b). Outdoors - On or about and times prior to February 25, 2015, a Gradall material handler (model: G6-42A) was operated with a non-functional reverse alarm, holes bored into both forks, and with a leaking hydraulic cylinder that controlled the tilt function, thus exposing employees to struck-by hazards.
