29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:      a) Job site, 7203 McKnight Road, Pittsburgh, PA: On or about February 27, 2015, during roofing activities on a low sloped roof, the employer did not ensure that employees were protected from falls of approximately 15 feet in that it was determined that warning lines along the rear roof area were located 5 feet from the edge of the roof and that parapet walls along the sides of the rear roof area were no higher than 32 inches tall.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Job site, 7203 McKnight Road, Pittsburgh, PA: On or about February 27, 2015, during roofing activities on a steep pitched roof, the employer did not ensure that employees were protected from falls of approximately at l east 15 feet in that the employer relied on parapet walls, determined to be as low as 24 inches from the roof in some places, as fall protection.

29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces and were not secured or were not provided with slip-resistant feet to prevent accidental displacement and slip resistant feet were used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that were constructed so they cannot be prevented from becoming slippery:   a) Job site, 7203 McKnight Road, Pittsburgh, PA: On or about February 27, 2015, during roofing activities, the employer did not ensure that ladder used to gain access to/egress from the roof was properly set up in that the ladder's feet were set up in an area of snow and ice accumulation and the ladder was not secured.
