The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor:           In the maintenance shop, southwest corner of the shop: On or about February 27, 2015, and at times prior thereto, employees did not have direct unobstructed exit route in case of an emergency situation that would require them to exit the facility.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:       In the kitchen, food mixing area: On or about February 27, 2015, and at times prior thereto, employees were exposed to an amputation hazard while using a Univex mixer that did not have a mixing guard for the rotating auger.

Abrasive wheels were not used only on machine provided with safety guards as defined in paragraph (c) (1) through (4) of this section:           In maintenance shop, tool storage area:  On or about February 27, 2015, and at times prior thereto, employees were exposed to a struck by hazard, while using a Bosch angle grinder that did not have a guard to protect the employees from getting struck by a piece of metal while cutting out walk-in doors on bathtubs.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    (a) In the laundry room, east wall: On or about February 27, 2015, and at times prior thereto, employees were exposed to an electrical hazard while washing laundry from a live electrical safety switch that did not have the cover engaged in the closed position to cover the exposed conductors.    (b) In the assisted living area, mechanical room: On or about February 27, 2015, and at times prior thereto, employees were exposed to an electrical hazard while adjusting a mechanical time light switch that had a missing electrical cover exposing electrical conductors.
