29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: a) On or about March 10, 2015 and times prior, employees were exposed to injury when using a 2inch by 12 foot nylon sling that was frayed (damaged) and should not have been in service. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) On or about March 10, 2015 and times prior, employees were exposed to injury when working above 1/2" re-bar that had not been capped exposing the sharp edges of the re-bar. The re-bar was located in and surrounding several holes above the creek in the flat work area.
