29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective: On or about February 26, 2015 at the Associated Wholesale Grocers facility-employees were exposed to Electrical Shock injuries, in that the electrical extension cord in use did not have a ground pin to ensure the path to ground was permanent, continuous and effective. Abatement certification is not required for this item.
