29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) West side, new residential construction located at 1805 Dearborne Lane, Alpharetta, GA 30009: Employees were exposed to severe laceration and amputation hazards while using a Ryobi 7-1/4-inch Circular Saw without the lower guard as a wood shim was placed between the lower guard and the upper guard to prevent the lower guard from binding while cutting the rafter tails.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: (a) West side, new residential construction located at 1805 Dearborne Lane, Alpharetta, GA 30009: Employees were exposed to falls of approximately 15-feet, 5-inches to the lower level while installing cornice boards from a ladder jack scaffold platform with no fall protection system in place.
