29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) Townhouses, Montague Way: An employee was elevated on four sheets of 8 foot by 4 foot, 1/2 inch thick plywood placed on the forks of a Pettibone (model B68, serial #8022) rough terrain forklift to a height of approximately 14 feet above ground level during the installation of windows. The Pettibone rough terrain forklift was not specifically designed by the manufacturer to elevate personnel, as determined on February 26, 2015. Abatement Certification Required for this Item.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) Townhouses, Montague Way: An employee was elevated on four sheets of 8 foot by 4 foot, 1/2 inch thick plywood placed on the forks of a Pettibone (model B68, serial #8022) rough terrain forklift to a height of approximately 14 feet above ground level during the installation of windows. The wooden sheet platform was not secured and attached to the forks, as determined on February 26, 2015. Abatement Certification Required for this Item.
