29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   35 New St, Worcester, MA  As of 3/12/15, the employer had not established a program consisting of energy control procedures, employee training, and periodic inspections to protect employees performing servicing and maintenance from startup or release of stored energy.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    35 New St, Worcester, MA  On 3/2/15 and at times prior, employees were exposed to the unguarded bending area pinch point of the StarTechnology automatic pipe bending machine.  On 1/13/15, an employee operating this machine had a fingertip amputated in that pinch point.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   35 New St, Worcester, MA  As of 3/2/15 at the start of the inspection, the employer had not reported to OSHA that on 1/13/15, an employee operating a machine received a fingertip amputation.
