(d) On or about 02/27/15 - # 1 Machine Shop, two Giddings & Lewis VTC 1600 mills had the front sliding door safety interlocks by-passed. (e) On or about 02/27/15 - # 1 Machine Shop, the Wilton pedestal drill press did not have chuck guards installed. (f) On or about 02/27/15 - # 1 Machine Shop, the Central table mounted drill press did not have chuck guards installed. (g) On or about 02/27/15 - # 2 Repair Bench, the red pedestal drill press did not have chuck guards installed.
