29 CFR 1910.23(a)(4):Skylights were not guarded by a standard skylight screen or a fixed standard railing on all exposed sides: 24 Wilson Way, Westwood, Ma.: Skylights did not have protective screens or were not protected by guardrails. A worker fell through a skylight while walking to an area on the roof to shovel snow and fell 23 feet to the concrete floor below.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: 24 Wilson Way, Westwood, Ma.: 1) Employees conducting snow removal were exposed to a fall of 23 feet from the edge of the room to the ground below. Workers were carrying tarps full of snow to the edge of a roof and dumping the snow. 2) Employees shoveling around skylights were not wearing fall protection and were exposed to a fall of 23 feet to the concrete floor below. A worker fell through a skylight and was seriously injured.
