29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury was not guarded. a) Jobsite: Warehouse - Employees were exposed to amputation hazards while operating a hydraulic press bailer machine as the point of operation was not guarded. An employee severed his arm after his clothing got caught in the point of operation. Observed on or about 2/24/15.
