Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to crush by hazards. a) In bay #1: On or about Feb 22, 2015 and at times prior thereto, an employee was utilizing a make shift pole to support a raised vehicle lift. Abatement certification and documentation ARE required for this item.
