29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    a) Jobsite, 1037 Vine Street., Philadelphia - Employees were lifted with windows in a platform by a material handler, up to approximately twenty five feet above ground level.  The employer did not ensure that the platform was designed by the manufacturer, exposing employees to a fall hazard.  Observed on or about 2/27/15.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a) Jobsite, 1037 Vine Street., Philadelphia - Employees were delivering windows, in a box platform lifted by a material handler up to approximately twenty five (25) feet above ground level.
