Omega Plastics, LLC, 1507 Wayne Street, Erie, PA 16503, maintenance shop / warehouse area, on or about March 3, 2015 and times prior thereto: An oxygen cylinder and an acetylene cylinder were stored within one-foot of each other on a welding cart. A noncombustible barrier was not present between the two cylinders.
