29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Production machines: employees were assigned to perform maintenance and servicing tasks when the hazardous energy control program (i.e. lockout tagout) was not established and implemented in the workplace. Training and periodic inspections were not conducted. Employees were exposed to hazards capable of causing hand(s) and finger(s) amputation from unexpected start-up.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed:    Production, cold heading machines: employees were assigned to perform setup and changeover tasks that exposed them to hand(s) and finger(s) caught-in between hazards capable of causing amputation. Procedures for machines testing or positioning (setup) in the point-of-operation and associated danger zones were not established in a sequence of actions required to maintain the integrity and continuity of employee protection as follows:    (a) clear machines of tools and materials;   (b) remove employees from the hazardous areas around the machine;   (c) remove the lockout/tagout devices;   (d) energize the machine and employ effective employee protection while testing or positioning machinery;   (e) and, turn off all systems, isolate the machine from the energy source, and reapply lockout/tagout devices as specified, if additional servicing or maintenance is required.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:    Production, cold heading machines: on or about February 3, 2015, employees were assigned to perform setup and changeover tasks that exposed them to hand(s) and finger(s) caught-in between hazards capable of causing amputation when equipment was not de-energized and other safeguarding was not provided.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
