29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. At this site, employees performed: a) cleaning of used oil and water based drilling mud from frac-tanks, dump trucks, and vacuum truck tanks; b) the spreading of used oil and based and water based mud with hand shovels; and, c) the transfer of used oil and water based drilling mud from one frac-tank into another. The employer did not provide employees with eye and face protection to protect from splashes such as safety goggles or face shields.
