29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Construction Site - On or about March 3, 2015 an extension cord did not have a grounding prong. An 11 Amp drill was plugged into the extension cord.
