29 CFR 1926.405(e)(2): Switches, circuit breakers, and switchboards installed in wet locations were not enclosed in weatherproof enclosures:  a) On or about March 3, 2015- Job site at 54-05 Skillman Ave, Flushing, NY 11377  The employer permitted their employees to use electrical equipment powered by an outdoor breaker box that was not   enclosed by a weatherproof enclosure.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  a) On or about March 3, 2015- job site at 54-05 Skillman Ave, Flushing, NY 11377  The employer permitted their employees to use flexible cord sets that were passed trough an opened window without   protection from damage.  These cords were used to power a portable electric heater and other office equipment.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about March 3, 2015- job site at 54-05 Skillman Ave, Flushing, NY 11377  The employer permitted their employees to work around an excavation, with two unprotected sides, approximately ten   feet above the next lower level without fall protection.  WRITEN ABATEMENT CERTIFICATION IS REQUIRED.
