29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: (a) Back Room - The flexible cord for the 220 volt temporary heater (number DK07110370) did not have an effective strain relief to prevent pull on the terminal screws. Employees were exposed to the hazards of electrical shock, electrocution on or about March 3, 2015. (b) Front Room - The flexible cord for the 220 volt temporary heater did not have an effective strain relief to prevent pull on the terminal screws. Employees were exposed to the hazards of electrical shock, electrocution on or about March 3, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
