29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. On or about February 17, 2015, and times prior thereto, above the bulk plant cement office, the employer stored materials without a load rating or a sign indicating the approved load limit exposing employees to struck-by or crushed-by hazards.

29 CFR 1910.36(e)(2): The door that connects any room to an exit route did not swing out in the direction of exit travel when the room is designed to be occupied by more than 50 people or if the room is a high hazard area: On or about February 17, 2015, and times prior thereto, inside the bulk plant cement building, the employer did not ensure that the three doors used by employees as exit routes swung outward in the direction of the exit discharge for quick exit of the building in case of an emergency exposing employees to entrapment hazards.
