Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards.    a) Laminator #5 - The employer did not ensure that the Caldwell Spreader Bar hooks were inspected prior to use and on a frequent basis.  The spreader bar hook was observed with a missing safety latch and not removed from service.         b) Press Department - The employer did not ensure that the Spreader Bar hooks were inspected prior to use and on a frequent basis.  The spreader bar hook was observed with a missing safety latch and not removed from service.        c) Laminator #4 - The employer did not ensure that the Caldwell Spreader Bar hooks were inspected prior to use and on a frequent basis.  The spreader bar hook was observed with a missing safety latch and not removed from service.            Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the guidelines in the American Society of Mechanical Engineers (ASME) B30.10-2014, Section 10.1.10 Inspection, Removal, and Repair.  The employer must ensure that inspections are being conducted on hooks and hooks found with damage shall be removed.  Repairs shall be approved by the manufacturer or a qualified person.
