29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: Facility: On March 5, 2015, employees were exposed to explosion hazards when Class II paper dust accumulation was being cleaned up by vacuum cleaners which were not intrinsically safe or approved for Class II Division E/F/G locations.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Third Floor, DBCS Machines: Maintenance employees were exposed to electrical shock and explosion hazards when they used a Pullman Holt Shop Vacuum (Model 45, 120 volt, serial #24667) to pick up Class II combustible dust which was missing its ground prong. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.304(g)(5), which was contained in OSHA inspection number 390144, citation number 02, item number 003 and was affirmed as a final order on July 30, 2012, with respect to a workplace located at 3131 Arch Airport Road, Stockton, CA 95213.
