29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer does not ensure that procedures are developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this section. This violation was observed on or about March 2, 2015, at the facility where employees were exposed to potential hazardous energy sources when performing set-up tasks on the Okuma CNC lathes without developing and documenting procedures for controlling hazardous energy for set-up tasks. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that procedures are developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this section.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: The employer does not ensure that authorized employees receive training. This violation was observed on or about March 2, 2015, at the facility where employees were exposed to potential hazardous energy sources when performing set-up tasks on the Okuma CNC lathes without providing authorized employee training for set-up tasks. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that authorized employees receive training.
