29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Jobsite located at 450 Snake Meadow Road, Moosup, Connecticut: On, or about, March 3, 2015, an employee suffered serious injuries when the hydraulic jack lifting a Clark/Michigan Model 275B-GM Wheel Loader slipped, causing the Wheel Loader to strike the employees upper back. The employer lacked procedures to ensure that all energy sources such as, but not limited to, gravity, had been controlled.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: Jobsite located at 450 Snake Meadow Road, Moosup, Connecticut: On, or about, March 3, 2015, an employee suffered serious injuries with the hydraulic jack lifting a Clark/Michigan Model 275B-GM Wheel Loader slipped, causing the wheel loader fall and to strike the employees upper back. The employer did not provide and ensure the proper use of appropriate blocks. Also, wheel chocks were not used in accordance with industry guidelines.

29 CFR 1910.244(a)(2)(vi): Each jack shall be thoroughly inspected at times which depend upon the service conditions. Inspections shall be not less frequent than the section (a) through (c) of the standard: Jobsite located at 450 Snake Meadow Road, Moosup, Connecticut: On, or about, March 3, 2015, the company had not developed and implemented a program of periodic inspections of jacks based on service condition and in accordance with manufacturer and industry guidelines. An Emerson jack assembly and an A.M.E. hydraulic jack had not been subject to inspections for the time they had been in service, which was estimated to be for at least several years.
