29 CFR 1926.403(b)(2): Listed, labeled, and/or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, and/or certification:    8 Ardsley Rd., Longmeadow, MA: On or about March 4, 2015, an employees was observed using a sliding compound miter saw outside of the residence that was powered through a Relocatable Power Tap (RPT) which;  a) was labeled to be used only indoors b) was powered through an extension cord  Neither of which is in accordance with the listing, labeling, and/or certification for this device.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) 8 Ardsley Rd., Longmeadow, MA: On or about March 4, 2015, an employee was working on the roof of this residence being constructed to clear an ice dam without any fall protection 12 feet above the ground below.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 8 Ardsley Rd., Longmeadow, MA: On or about March 4, 2015, an employee was observed climbing from a ladder, that did not extend the required 3 feet past the upper landing surface (roof) that he was accessing to perform his work.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
