29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) At this job site, Intersection of N. Loop 1604 E. and Nacogdoches Road, employee was exposed to struck-by hazards, from items such as but not limited to: utility knife, wrench and/or hammer, when working with no protective helmet while standing below an employee working above him from an Altec aerial lift, while installing an advertising banner. (b) At this job site, Intersection of N. Loop 1604 E. and Nacogdoches Road, employee was exposed to fall hazards greater than 10 feet, when working with no fall protection from an Altec aerial lift, while installing an advertising banner.
