One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On 3/02/2015, employees from Lucent Window & Door Co., LLC were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  In the maintenance area, a MAXTURN Lathe, KBC Milling Machine and a MSC Milling Machine, serial # 760415, did not have guards.

29 CFR 1910.215(b)(9):   Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.  On 3/02/2015, employees from Lucent Window & Door Co., LLC were exposed to a struck-by hazard. In the maintenance area, a Dayton 6bench grinder, model # 4Z123H, did not have the tongue guards adjusted to ensure the diameter never exceeded one-fourth inch.

Throughout the plant, multiple junction boxes had circuit breakers to control lighting.  There were no switches in the circuits controlled by the circuit breakers to turn on or off the overhead lights.

Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, unless located and arranged so the purpose was evident.  On 3/02/2015, Lucent Window & Door Co., LLC had not ensured each overcurrent device was legibly marked to indicate its purpose.  Several junction boxes in the plant did not have markings to indicate the purpose of the circuit breakers.
