29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  a)  At the jobsite, on the first floor, on or about 3/4/15:  Employees were rip-cutting material on a table saw without the blade being guarded.  Abatement certification must be submitted for this item.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.  a)  At the jobsite, on the 3rd floor, on or about 3/4/15:  An employee back fed a circuit by inserting conductive bare wire into the female end of an extension cord that was powered by the temporary service.    b)  At the jobsite, on the 1st floor, on or about 3/4/15:  Employees were working and transiting the stairwell to the upper floors where a box fan designed for household purposes had been installed and used to move warm air upstairs.    Abatement certification must be submitted for this item.

Abatement certification must be submitted for this item.
